Guarantor

Guarantor Reminder:
Many incorrect guarantors are being entered in the system. Remember that the guarantor can be different than the subscriber of the insurance and should match the name of the person signing the consent form.  Anyone 18 and over is legally their own guarantor unless they are not ‘of sound mind’ and someone has power of attorney (POA) for them.  If this is the situation we must have a copy of the POA paperwork that has been filed with the court.   In the case of minors remember the guarantor is the parent bringing them in for treatment who signs the consent form.
