Addendums
When a clinician signs documentation, it is locked and can no longer be changed. However, edits can be entered in an Addendum to the document.
**Note: only the clinician who created the document can enter an addendum.
- From the patient’s case Home Page, click the Documentation button.
- Click the magnifying glass icon to the right of the document to open a “read only” version.

There are 2 ways to get to the addendum section:
- Click on Go to addendums in the header of the Daily Note, Plan of Care, or Progress Note   
- Scroll to the bottom of the document (under the Signature section) and click the New Addendum button  
- Enter the addendum information in the Description field. Please try to be as specific and detailed as possible so documentation will hold up with insurance if needed.
- A Progress Note Addendum includes options to:
  - Change Visits/ Frequency
- Place a Patient's Case on Hold
    - Click the checkbox next o Change visits/ frequency or put patient on hold  
- You will then see the options to:
      - Update the Plan of Care Status
- Place the Patient's case on hold
- Update the Frequency and Duration  
- If placing a patient on hold make sure to enter a reason why in the Addendum Description area.  
 
 
 
- After entering Addendum information, enter your 4-Digit PIN and click Sign and Submit  
- The Addendum will now show at the bottom of the HTML version and the PDF version. Please see examples below.
