Patient Case: Referral Tab

AgileEMR: Case – Referral Tab

FEATURE PURPOSE: Allows the user to input detailed information about the patient’s referring physician and diagnosis for that case.

If the patient is a Self-Referral, use the "Self Referral MD" physician under Primary Referring Physician field and fill in the rest of the information as shown below
If the patient is not a self-referral, fill in the appropriate information as below. If you happen to miss a field that is required, the referral tab will show a red alert with a number, indicating how many fields remain empty that need to be filled in.

Required Information:

Primary Referring Physician – Whomever referred the patient to the clinic
- If the referring physician is not already stored within AgileRPM, please contact Agile Support, or reach out to an FOC Team Lead to have them add this for you.

- The Referral Expiration Date will automatically populate after the Referral Date has been entered.
Additional Referring Physicians
- If the patient has multiple referring physicians, you can enter their information here, BUT the system will only fax doucments to the physician listed in the Primary Physician Field. If you need to send it to a different physician, then you will

- Recertification Date, Privacy Date, and the Assignments of Benefits Date will populate automatically with whatever date you enter into the Evaluation Date field.

Documentation vs. Non-Documentation: There is a slight difference between Documentation Referral Tab layout and Non-Documentation Referral Tab layout. The Non-Documentation Referral Tab is also going to require that you enter the Primary Diagnosis Code. It will not allow you to proceed until this has been filled in. Users can also add any additional diagnosis codes as needed. Documentation cases will not require this information in the Referral Tab.
