AgileEMR: Adding a Patient Case to Agile

FEATURE PURPOSE: Allows users to create new cases for new patients/returning patients, etc.

SEARCHING FOR AN EXISTING PATIENT

  • This can be done from the Case List or the Patient List
  • If you are searching for a Patient's name within the Patient List, then you will not be able to locate them if the case has been discharged.
    • If you are unsure if the patient has been discharged, use the Case List as it will display all cases even if they are discharged.

CREATING A NEW CASE FOR A NEW PATIENT

  • Click on Cases or Patients in the top Menu Bar within Agile. (Patient cases can also be created from the Agile PM Welcome Page upon logging in).

  • On the far left there should be an option for + New Case, click here to begin adding the patients case into the system.







  • Select the Case Type: will it require documentation, or if it will be a non-documentation case

  • Once selected, begin filling in Patient's information (Required information: First Name, Last Name, Birthdate & Home Phone Number)
    • Be sure that you do not include any special characters in any of the fields, as this can cause issues on the backend with claims.
    • If Patient has two last names, or multiple names do not use a hyphen to separate them. For Example, if you have a Maria Rosales-Gonzales then it should be Maria RosalesGonzales

  • Hit GO once entered and the system will search to see if this person already exists in the system. It will provide you with a list of similar names, so that you do not create a second case for the same patient.
  • If you do not see the patient's name in this list, you can select the Create New Patient option and this will create the case in the system.

  • After clicking Create New Patient, the system should automatically open the case and take you into the Patient tab. This is where you would enter the patient details.

ADDING A PATIENT CASE TO AN ALREADY EXISTING CASE

  • If the patient that is being seen gets a new script for a different body region, you will want to create a new case for the new referral
  • Navigate to the Patient's existing case
  • Click on the Magnifying Glass next to their name at the top of the Home Tab.

  • Click on New Case in the upper left-hand corner and the system will allow you to create a documentation/non-documentation case under the same patient name/MRN.


DELETING A PATIENT CASE

  • The only time that Agile will allow you to completely delete a Patients case, is if you delete it immediately after creation.
    • If you add an appointment to the case, you will no longer have the option to delete the case
    • When this happens, you would first want to remove the appointment from the schedule and then discharge the case.


DISCHARGING A PATIENT CASE

  • Navigate to the patients case within Agile
  • Look at the bottom left under the list of tabs, in Actions you should see an option for Discharge
  • Click on the Discharge button and it will open a new window.
  • In this new window, Enter Discharge Reason and Discharge Date.
  • Once entered, Click on Discharge Case

REOPENING A PATIENT CASE

If a patient has been discharged prematurely, and they return at a later date for more Physical Therapy, you are also able to reopen a discharged case.

  • First navigate to the patient's case
  • Look in the bottom left of the Patient's case and instead of "Discharge" you should see "Re-Open Case"
  • The system will prompt and ask you if you are sure you would like to re-open the case, Select "Re-Open Case" 

NOTE: If you attempt to "Re-Open" or "Discharge" a case and the system gives you a warning, then please send in a support ticket and a support representative will troubleshoot the issue for you.
