Placing a Case on Hold via Progress Note
FEATURE PURPOSE: To allow therapists to place a patient case on Hold if the patient will be absent for more than two weeks. This action prevents a PN or DS task from populating to the Therapist Work Queue and allows a therapist to place the case on hold for a certain duration.
Therapist can choose to place a Patients Case on Hold for the following reasons:
- Vacation
- Surgery
- Travel
- Physician Ordered
- Hospitalization
- Other
Placing the Case on Hold:
- To place a case on hold, navigate to the Documentation tab of the patient’s case and select Progress Note
- Complete the required fields in the Progress Note
- Scroll to the bottom of the page and select Put patient on hold
**NOTE: If there are remaining visits left that have been scheduled, the system will prompt an alert allowing the user to continue with placing the case on hold or to cancel. If the user continues, the scheduled visits will be cancelled.
- Select the Hold Until date
- **Note: Users can place a case on hold up to 3 months)
- Sign and Submit the document
- A Case Hold – PN task will display in the Therapist Work Queue once the case has been placed on hold
Release a case from hold and request additional visits or discharge the case:
- Navigate to the work queue, select Open Document for the Case Hold – PN task
- Once selected, this will direct the user to the Progress Note Addendum section
- Select New Addendum
- Select Release Patient from Hold
- At this time the user can either request additional visits or discharge the case
- If additional visits are requested, the Frequency and Duration MUST be updated
- If Discharging the case, user will need to select the Discharge Plan
- Provide reason for addendum, Sign and Submit
