Changing /Removing Diagnosis Codes on Initial Evaluation

• Go to the patient’s case Documentation page and click the spyglass next to the Initial Evaluation

• Select the arrow to expand on the Signature section and select Open & Edit Initial Evaluation & Plan of Care

• Select the Diagnosis section to open and edit the diagnosis codes

• Select the Trash Can icon next to the Diagnosis you want to remove 

• Add the new ICD-10 diagnosis code by pressing Select

• Use the Keyword Search to find the correct diagnosis code

• Find the Diagnosis from the list and click Use This Code:

• Save Changes

• Go back to Signature section to sign the document and provide reason for edit. The Initial Evaluation MUST be re-signed, or it will remain open until it is.
