Creating the Plan of Care

You will create the  Initial Plan of Care while completing the Initial Evaluation for the patient. 

  • Open the Initial Evaluation document
  • Go to the Plan section of the Initial Evaluation

**NOTE: You will need to complete the rest of the Initial Evaluation before a Plan of Care will be generated.

  • In the Plan section you will be able to:
    • List the total number of visits for the case
    • Define Frequency and Duration of visits
    • List additional recommendations regarding Visits/ Plan of Care for the patient
  • In the Signature section of the Initial Evaluation, you will be able to preview the PDF version of the Plan of Care before signing the document by clicking on the Preview Plan of Care button.

  • Once the Initial Evaluation is created and signed, a separate Plan of Care document will be generated from the Plan section of the Initial Evaluation

Updating the Initial Evaluation & Plan of Care:

**NOTE: You can ONLY update the Initial Evaluation & Plan of Care if no other documents have been created yet. 

  • Go to the Documentation tab of AgileRPM
  • Click the spy glass next to the Plan of Care document or the Initial Evaluation document under the Signed Documents header
  • In the Signature section at the bottom of the document click on Open & Edit Initial Evaluation & Plan of Care 
  • The entire Initial Evaluation & Plan of Care will open for editing and re-sign. 
  • Make any corrections/ modifications as necessary to the Initial Evaluation & Plan of Care
  • Provide a Reason for Edit in the field provided and re-sign the document
  • Check the Patient Agreement section stating that the Plan & Goals for treatment has been discussed with the patient
