Update Plan of Care Tasks

There are 4 different tasks associated with the Plan of Care (POC)/ Progress Note (PN)

  • Expired Plan of Care- this means that your visits or duration from your Plan of Care has expired.
  • Expiring Plan of Care- this means that your visits or duration from your Plan of Care is expiring. 
  • Progress Note Due- this means that according to the State or Payer guidelines attached to the case, you are required to complete a Progress Note for insurance purposes
  • Upcoming Progress Note- this means that according to the State or Payer guidelines attached to the case, you will need to complete a Progress Note soon for insurance purposes. 

 Plan of Care & Progress Note Tasks

  • If you fill out an Updated Plan of Care at any time, the Progress Note rules will Restart from the date of the Update Plan of Care.

Clinician Work Queue Tasks

Clinicians tasks are divided into three options: 

  • Due
  • Upcoming Tasks
  • All

You can change this view by clicking the appropriate button in the blue Therapist Work Queue header.

You will also have these view options : 

  • View All 
  • My Assignments
  • My Cases
  • Switch to Assistant Queue
  • Switch to Student Queue 

You'll notice a column named Remaining Visits/ Days. This is meant to help the clinician understand why a Progress Note or Plan of Care task has generated. These columns will tell you:

  • How many visits or days remain until either the Progress Note (PN) is upcoming or the Plan of Care (POC) is expiring.
  • How many days have passed since the Progress note was due or the Plan of Care was expired. 
  • How many days have passed since the Daily Note (DN) or Discharge Summary (DS) tasks have been in the queue since generated

Schedule

You'll notice a POC next to the patient's appointment times if the Plan of Care is Expired or Expiring. 

  • If this notice is YELLOW a Plan of Care is Expiring and a new one will need to be completed soon
  • If this notice is RED that means the Plan of Care for that patient is Expired and an Updated Plan of Care or Progress Note will have to be completed at today's visit. 

You can also check the status of a patient's Plan of Care by clicking on their appointment. 

In the appointment pop up you will see a Plan of Care/ PN Status table in the Documentation section for your review.
