Charge/ Billing Corrections

**NOTE: When a Daily Note has been signed by the clinician AND the charges are submitted to billing, charge edits can no longer be made within the Daily Note documentation. However you can add an Addendum on the bottom of the Daily Note documentation and inform the appropriate party for this to be corrected. 

Clinician: 

**Note: Only the treating clinician should be entering Addendums.

  • Go to patient's case 
  • Go to the Documentation tab
  • Open the note for that date of service
  • Click the Add Addendum button (review the Documentation Addendums document to learn how to enter an Addendum to a document)
  • Enter all the necessary modifications that need to be made (Be sure to include the CPT/ Charge Code, minutes, and units
  • Save the document

Who & When to Notify:

  • If charges were entered on the current day, Notify the Front Office Coordinator (FOC) of changes 
  • If the charges were entered on a past date, Notify AgileRPM at support@agilerpm.com of the Addendum entered and the date of service, asking them to correct the charges per the Addendum.

**An addendum by the treating clinician needs to be entered before any billing is changed. 

Clerical/ Front Office:

  • Go to the patient's case
  • Go to the Documentation tab
  • Open the Daily Note for the date of service as directed by the clinician
  • Review the Addendum added to the note by the treating clinician
  • Open the Account tab in a new web tab
  • Click on the magnifying glass next to the date of service to open it

  • Click the Trash Can on the far right of the CPT code that needs to be removed

**Note: The Trash Can will only show when the Status is Created. If the Status is billed a ticket will need to be sent to AgileRPM support at support@agilerpm.com for charges to be corrected.

To add a new charge to a date of service that is in the Status of Created and not billed.

**Note: Supplies can also be entered here

  • Select the CPT/ Charge code in the New Charges section of the Account tab.

*Hint: You can start typing in the drop-down box where it says select to find the CPT codes you're looking for more easily. 

  • Once a CPT code is selected in the New Charges section of the Account tab you will be able to: 
    • Update the number of units to bill for that CPT
    • Select if that code should be a Self Pay treatment
    • Add a Modifier if necessary
    • Delete the CPT code if the incorrect CPT code was selected
    • Add or Remove diagnosis codes for the CPT code selected

  • List the correct number of units to bill for that CPT code

  • Click the Self Pay checkbox if this needs to be billed to the patient as a Self Pay treatment 

  • List any Modifiers needed for that CPT code in the Modifier field

  • Click the Trash Can to remove the CPT code selected

To Add a Diagnosis code: 
  • Click on the + next to the diagnosis box in the New Charges section

  • You'll then see the Select Code for Additional Diagnosis pop up window

  • Start typing the Diagnosis Code/ Chief Complaint in the Keyword Search box 
    • This will give you the CPT Code section that the specified diagnosis code falls under

  • Click the arrow to expand the code sections until you get to the Blue code, as that will be the most specific code for the diagnosis/ chief complaint. 
    • You will not be able to select the codes leading up to the blue code as they are not specific enough to be billed out. 
  • Click on the Blue code to select the diagnosis for billing

  • After clicking on the Blue code you will see Instructional notes pop up on the right

  • To use/ add this diagnosis code, click on Use this Code
  • The selected diagnosis will then be added under the Diagnosis field in the New Charges section of the Account tab

To Remove a Diagnosis code: 
  • Click on the X next to the Diagnosis code in the Diagnosis field of the New Charges section of the Account tab

  • This will remove the Diagnosis code from the list and not apply it to the CPT code. 
  • Click Submit to add the CPT for billing
